A Care Manager will usually contact us on the person’s behalf with a copy of the individual’s care plan to review. This provides us with vital information regarding the needs of the person. Before we offer a service to any individual we will arrange to carry out an assessment of need. This is normally carried out in the person’s own home and doing this visit we will be able to ascertain what sort of support the person wants and needs and whether we can provide the right service.
